Map form headers to card description

Would be great to be able to map headers from forms onto action cards - right now these are only visible on the form and not on the action card description that the dorm creates. For forms that contain multiple requests / request types, these headers are vital to keeping information organised within a project.

When testing this, it appears that there may need to be review on this. We have hive forms that are broken into sections, there are so many questions wanted to give our clients a break. When testing is shows the header for section 1, but when they answer no to moving on the next section the headers are not sent with the reply. When going back and moving on to section 2 it shows header text for Section 1 and Section 2 but not the other header texts. Thoughts as to why it does this?

This would be very helpful for us too. We have an intake form that has multiple sections depending on user selection, and if the user selects multiple options, we get the responses all lumped together in the action description without easily being able to tell which response belongs to which section.
